It is a permanent method of birth control surgical contraception or voluntary sterilization. It is a surgical method where by the reproductive function of an individual Male or Female is purposefully and permanently destroyed. The operation done on Male is Vasectomy and that on the Female is Tubal Occlusion, or Tubectomy.

It is a permanent Male sterilization operation done in the Male, where a segment of Vas deferens of both the sides is resected and the cut ends are legated.
• Operation can be done as an outdoor procedure
• Failure rate is minimal
• Vasectomies are easier and less expensive
• Permanent methods of contraception are only for people who do not want children now or in future

• Stretched skin over the Vas is punctured with the sharp pointed end of a forceps instead of using a scalpel
• Then the holes are increased and the Vas is dissected out by using the tips of the forceps
• This method is effective and reversible method
• Polyurethane Elastomer is injected into the Vas
• It solidifies and forms a plug and blocks the sperm passage
• The plugs can be removed under Local anaesthetic
• The abdomen end of the resected Vas is coagulated
• The testicular end is left open
• This will prevent Congestive Epididymitis
• A Vasectomy is usually performed by a doctor who is specialized in the Male urinary tract and reproductive system
• In some cases, the physician may decide to do a Vasectomy in an outpatient surgery center or a hospital. There are two ways to perform a Vasectomy. In either case, the patient is awake during the procedure, so the Surgeon uses a Local anesthetic to numb the scrotum
• With the conventional method, the Surgeon makes one or two small cuts in the scrotum to access the Vas deferens
• A small section of the Vas deferens is cut out and then removed
• The Surgeon will perform the same procedure on the other testicle, either through the same opening or through a second scrotal incision
• For both the testicles, when the Vas deferens has been tied off, the Surgeon will use a few stitches or skin to close the opening in the scrotum
No Scalpel method
• With the "No-Scalpel" method, a small puncture hole is made on one side of the scrotum. The Surgeon will find the Vas deferens under the skin and pull it through the hole
• The Vas deferens is then cut and a small section is removed
• The ends are either sealed with heat or tied off and then put back in place
• The procedure is then performed on the other testicle. No stitches are needed with this method because the puncture holes are so small
• After a Vasectomy, most men go home the same day and fully recover in less than a week

Tubectomy is also called as Tubal Ligation and this surgical procedure is carried out to ensure that the Fallopian Tubes in your body are clamped or blocked, sealed or severed. The main aim of Tubectomy surgery is to prevent the sperm from reaching the egg to fertilize it.

• Laparoscopic Sterilization is performed under General anesthesia or Spinal anesthesia or Local anesthesia
• The actual procedure is done in an operating room, either in a hospital or a surgical center
• The surgery takes about half an hour
• In Laparoscopy procedure, the abdomen is filled with CO2 gas by introducing a needle, so that the abdominal wall balloons away from the uterus and tubes
• The Surgeon makes a small cut just below the navel or umbilical area and inserts a laparoscope, a small telescope like instrument
• A second incision is made just above the pubic hairline to insert another small instrument that can help with closure of the Fallopian Tubes
• Usually, Filshie clips or Falope rings are placed on the Fallopian Tubes to block the tubes. Sometimes the tubes are clipped and cut
• The skin incision is then closed with minimum stitch or a tape. The patient may feel well enough to go home from the outpatient surgery center in a few hours
• Minilaparotomy or Periumbilical Minilaparotomy is the most common procedure used immediately after childbirth
• Postpartum Tubal Ligation is technically simple because the immediate after delivery, the uterine fundus is at the level of the umbilicus, making the Fallopian Tubes readily accessible through a small Periumbilical Abdominal incision
• Minilaparotomy following delivery in the early Puerperium is convenient, it is simple, and Minilaparotomy is cost effective. However, if maternal or infant complications exist, sterilization should be delayed
• Laparotomy (concurrent with Cesarean delivery)
• Laparotomy is an incision made in the abdominal wall to allow a Surgeon to look at the organs. Bilateral Tube Ligation may be performed after closure of the uterine
• This procedure is done under General or Spinal anesthesia
• The Surgeon can get it done only by conversant with Vaginal plastic operation
• It can only be done in the interval period or following delivery or abortion provided, the uterus is smaller than 12 weeks
• The operation can take longer time
